Audiograms
Audiogram hours are from 8 a.m. to 3 p.m., Monday - Tuesday and Thursday-Friday. Audiograms are not conduced on Wednesdays, as we are closed for administration/training time. Personnel can call 813-827-9601 to get scheduled for the following audiograms:
- Reference
- Annual
- Pre-employment
- Pre-and-post-deployment
- Separation/retirement
All other audiograms are to be scheduled with the audiologist.
- For personnel assigned to industrial shops, all "Occ and Environmental Health Exposure Data" as seen on your IMR will be uploaded by public health technicians. No appointment is required.
- For newly assigned occupational shot supervisors, please call 813-827-9601 and ask for a member of the Occupational Health Clinic to receive shop supervisor training.
