Audiogram hours are from 8 a.m. to 3 p.m., Monday, Tuesday, Thursday, and Friday.
- Request an appointment for your reference, annual, pre-employment, pre- or post-deployment, or separation or retirement audiograms using the QR code or by viewing the Request an Audiogram Appointment page.
Note: Audiograms requested through the outpatient app are not actual appointments until confirmation message is sent to patient.
- For personnel assigned to industrial shops, all “Occupational and Environmental Health Exposure Data” as seen on your Individual Medical Readiness, or IMR, will be uploaded by public health technicians; therefore, no appointment is required.
- For newly assigned occupational shop supervisors, please call 813-827-9601 and ask for a member of the Occupational Health Clinic to receive shop supervisor training information.
